The report is not the work. The discipline is.

An 8D written the night before it is due is a document, not an investigation. The steps exist in an order for a reason: containment protects the customer while the cause is still unknown, and effectiveness verification is what separates a fix from a hope.

Kept as a form, an 8D repeats. Kept as a record that links to the nonconformance that started it, the FMEA row that should have predicted it and the control plan that has to change, it closes the loop it was designed to close.

D1 to D8, in order

Containment first

What protects the customer today is recorded before the cause is known, which is the point of D3.

Fishbone and five-why held as structured data rather than as a picture pasted into a document.

D6 is not closed by assertion — the evidence that the action worked is part of the record.

D7 links to the FMEA and control plan changes, so prevention is a change to the system rather than a sentence.

An 8D starts at a nonconformance and ends in the FMEA.

The complaint or NCR is D2. The prevention step is a change to the risk analysis and the control plan. An 8D that does not end there has not prevented anything.

Questions about 8D software

What are the eight disciplines?
D1 team, D2 problem description, D3 interim containment, D4 root cause, D5 chosen corrective action, D6 implementation and validation, D7 prevention of recurrence, D8 recognition and closure. The order matters: containment comes before cause because the customer cannot wait for the investigation.
What is the difference between 8D and CAPA?
They overlap. 8D is a specific eight-step format, usually raised in response to a customer complaint and returned to that customer. CAPA is the broader corrective and preventive action process that may be triggered internally. The same investigation can be run either way; 8D is the one with a format the customer expects.
Can we attach evidence to each discipline?
Yes. Photographs, measurement data and documents attach to the step they belong to, so the report and its evidence stay together.
Does closing an 8D change the FMEA automatically?
No, and it should not. It raises the change and links to it — a person still decides what the new risk analysis says. Automatic edits to a controlled risk document is not something any auditor would accept.
