An FMEA goes stale in a specific way. The process changes, the control plan gets updated because the shop needs it to be right, and the FMEA — which is the document that was supposed to justify the control — is updated last, or not at all. By the next audit the two documents describe different factories.

When the FMEA and the control plan are built on the same operation list, that drift becomes visible instead of invisible. And when the FMEA is a controlled document with a revision and a review state, "we updated it" is a fact with a date on it rather than a claim.

Questions about FMEA software

What is an FMEA?
Failure Mode and Effects Analysis is a structured way of asking what could go wrong, how bad it would be, how likely it is, and whether you would catch it — then doing something about the combinations that matter most. It is done before production, and revisited whenever the design or the process changes.
What is the difference between DFMEA and PFMEA?
A DFMEA analyses the design: what could fail about the part itself, and what the design does to prevent it. A PFMEA analyses the manufacturing process: what could go wrong at each operation, and which control would catch it. Both are supported, against the same part.
Does the software use Action Priority or RPN?
The AIAG-VDA structure is implemented, in which Action Priority is the evaluation output. The rating tables that drive any such evaluation are licensed reference material — the software provides the structure and the workflow, and your licensed tables are data you load rather than content shipped inside the product.
Can an approved FMEA be edited?
Not in place. A released FMEA is superseded by a new revision, which is the point of revision control — the version somebody approved stays exactly as they approved it, and the change has a reason and a date.
Is the FMEA connected to the control plan?
Yes, through the process operations both are built on. Detection controls identified in the PFMEA carry through into the control plan rather than being re-typed into it.
